Sleeping Assessment
Overview
This assessment asks you to gather information that gives us a full picture of your child’s sleep. As this is a comprehensive process, it may take you some time to complete. It will also take some time for me to review. Once I have finished reviewing your assessment materials, I will invite you to schedule our assessment meeting.
1. Child’s Sleeping History and Development Questionnaire
(~30 minutes to complete)
This questionnaire addresses your child and family from a historical as well as a present-day perspective. It will help you remember significant events and allow me to get a feeling for your child and family.
2. Child Sleep Competence Questionnaire
(~30 minutes to complete)
This questionnaire asks about how you currently help your child sleep and how you think and feel about their sleep. Please have each person who regularly helps the child sleep fill out this questionnaire.
3. Adult Sleep Competence Questionnaire
(~30 minutes to complete)
This questionnaire asks about how you, as an adult, sleep and feel about your sleep. Please have each person who regularly helps the child sleep fill out this questionnaire.
4. Seven-Day Sleep Log
You will use this log to record your child’s sleep every day for a week. You can print out the log, create your own document and fill it out digitally, or you can copy the chart into a physical journal and submit photos of the pages.
5. Videos of Sleep Routine
Videos of your child’s sleep routine will help us notice details that will guide your child’s future sleep plan. At least one video is required for us to do our assessment, but I recommend two or three videos.
6. Permission to Share Your Story (Optional)

This waiver invites you to share videos, photos, and/or quotes from your assessment and guidance to help other families and professionals who are struggling with children’s sleep.
